WHAT PERSONAL DATA WE COLLECT AND WHY WE COLLECT IT

Personal data means any information about an individual from which that person can be identified.

We collect information about you during the booking process on our appointment system.  This information may include, but is not limited to:

·  Identity Data - includes first name, last name, any previous names, username or similar identifier, marital 

   status, title, date of birth and gender

·  Contact Data - includes home address, email address and telephone number

·  Profile Data - includes bookings made by you, your clinical diagnoses, your preferences,

   feedback and survey responses

·  Usage Data - includes information about how you interact with and use our website and services


HOW WE USE YOUR PERSONAL DATA

Handling this data allows us to:

·  Send you important appointment and clinical information

·  Respond to your queries or complaints

·  Process payments and to prevent fraudulent transactions. We do this on the basis of

   our legitimate business interests

·  Set up and administer your account, and to verify your identity


Additionally, we may also collect the following information:

·  Location and traffic data (including IP address and browser type) if you book an appointment 

   through our booking system

·  Website pages visited and content viewed whilst your session is active


HOW WE COLLECT YOUR PERSONAL DATA

We use different methods to collect data about you including completion of contact form, clinical diagnoses and email address.  We will never use the information submitted for marketing purposes or sell your data to third parties.


WHO WE SHARE YOUR DATA WITH

We use Cliniko (practice management software designed for allied health practitioners) for our patient booking system and to store your electronic clinical data.


COOKIES

Our website uses cookies for essential website functionality and to enhance your experience.  You can manage cookie preferences through your browser settings.


DATA RETENTION

We will only retain your personal data for as long as reasonably necessary to fulfil the purposes for which we collected it, including for the purposes of satisfying any legal, regulatory, tax, accounting, or reporting requirements. We may retain your personal data for a longer period in the event of a complaint or if we reasonably believe there is a prospect of litigation in respect of our relationship with you.


By law we must keep basic information about patient care (including Contact, Identity, and Clinical Data) for seven years after they cease being a patient.  


YOUR LEGAL RIGHTS


Unless subject to an exemption under the data protection laws, you have the following rights with respect to your personal data:


·  The right to request a copy of the personal data which we hold about you

·  The right to request that we correct any personal data if it is found to be inaccurate or out of date

·  The right to request your personal data is erased where it is no longer necessary to retain such data

·  The right to withdraw your consent to the processing at any time, where consent was the lawful basis 

   for processing your data

·  The right to request that we provide you with your personal data and where possible, to transmit that 

   data directly to another data controller, (known as the right to data portability), where applicable i.e.

   where our processing is based on consent or is necessary for the performance of our contract with

   you or where we process your data by automated means

·  The right, where there is a dispute in relation to the accuracy or processing of your personal data, to

   request a restriction is placed on further processing

·  The right to object to our processing of personal data, where applicable i.e. where processing is based on

   our legitimate interests (or in performance of a task in the public interest/exercise of official authority)

We may update this privacy policy from time to time in order to reflect changes to our practices or for other operational, legal, or regulatory reasons.  Version control is exercised and noted at the end of this document.

You have the right to make a complaint at any time to the Information Commissioner’s Office (ICO) who is the UK regulator for data protection issues (www.ico.org.uk).
